$5.00 per month equipment rental fee per receiver. $4.99 programming access fee will be waived monthly for each receiver continuously connected to the Customer's phone line each month.  Requires twelve-month agreement to at least the America's Top 60 or DISH Latino packages. Customers activating a high definition receiver must also agree to purchase the $9.99/mo. HD pack for the duration of their service agreement.

To receive a model 411 or ViP211 receiver, a new customer activated under this promotional program must pay a non-refundable $49.99 lease upgrade fee.
To receive a second model 625/522 receiver, a new customer activated under this promotional program must pay a non-refundable $99 lease upgrade fee.
To receive a model ViP622 DVR receiver, a new customer activated under this promotional program must pay a non-refundable $299.99 lease upgrade fee.
